Under general supervision, evaluates, plans and implements the treatment of home care patients through the use of various modalities such as heat, light, water, sound, therapeutic exercises, adaptive devices, and ambulation training. Monitors Physical Therapy Assistants, Restorative Aides, and Home Health Aides in the implementation of care plans. Responsible for the delivery of age appropriate care for assigned child, adolescent, adult and geriatric patients with a wide variety of medical surgical conditions.
Conducts initial evaluations on referred patients and prescribes treatment plans in accordance with department standards.
Schedules patients for treatment in accordance with physician referral, patient assessment and treatment plan.
Conducts treatment sessions.
Performs patient assessments including: biophysical, psychosocial, spiritual, cultural, and age appropriate needs of the patients, including the assessment for signs/symptoms of abuse or neglect in order to develop, evaluate, and modify patient's plan of care. In conjunction with other disciplines, develops a realistic comprehensive patient/family educational plan, using appropriate resources and methods of instruction and provide patient/family health teaching.
Maintains records of all treatments procedures, including initial evaluations, progress notes, discharge summaries, and daily documentation.
Evaluates patient progress and makes necessary revisions to treatment plan in order to meet the patient's needs and treatment team goals.
Consults with physicians, family members, and other rehabilitation personnel to assist in coordinating the patient's total rehabilitation.
Provides education and training to patients and family members in order to maximize patient's independence at home and in the community.
Assists in the training and supervision of clinical students, and supports treatment plans being carried out by other disciplines as part of the treatment team goals.
Performs related duties to include participating in: special committee work as assigned, quality assessment, and improvement monitoring.
Bachelor's Degree in Physical Therapy.
Current Missouri PT license and valid driver's license.

Must maintain a valid driver's license and a driver's record deemed acceptable by SSM. Must maintain and be able to produce proof of current auto insurance with at least state required minimum coverage.
This position requires daily travel to patient homes and meeting locations, the ability to operate a vehicle in a safe and lawful manner is an essential function of the job. Additionally, access to a vehicle in good working condition required.
